Facility Evaluation Report
Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced Case Management - Incident visit. LPA identified herself to, was greeted by, and explained the purpose of the visit to Marshelle Gibson, DSP Staff member. Later Director of Consumer Services Diana Spurgeo joined the visit.
Community Care Licensing received an Incident Report on 10/20/2025 in which it was reported that on 10/17/2025 Client #1 (C1) returned from a dental appointment with a small bruise to the shoulder which grew into a large bruise extending down to arm. [See LIC811 Confidential Name List for a description of select person identifiers used in this report.] C1's Responsible Party and Primary Care Physician (PCP) were notified.
LPA performed a facility tour, welfare check, and interviewed staff. Staff interviews corroborated the events of the report. A health and safety visit with C1 was also conducted.
No Deficiencies were cited during the visit as the facility responded accordingly and appropriately.
An exit interview was conducted with Director of Consumer Services Diana Spurgeon, with whom a copy of this report was provided. Their signature below confirms receipt of this document.
