Facility Evaluation Report

Licensing Program Analyst (LPA) Arian Golbakhsh conducted an unannounced Case Management visit. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to caregiver Reynerio Millan and Administrator Alice Millan.

The facility is planning a change of location and undergoing the process with the Department to license the new facility location. The purpose of LPA's visit was to conduct an inventory of furniture items that are planned be moved to the facility's new location should their license be approved.

LPA, accompanied with Administrator Millan, toured the facility and inspected each room. LPA verified that all required furniture items were present and in good repair.

No deficiencies were cited during today's visit. An exit interview was conducted with Administrator Millan to whom a copy of this report was provided. Their signature below confirms receipt of this document.
