Facility Evaluation Report

Licensing Program Analyst (LPA) Arian Golbakhsh conducted an unannounced Case Management visit to obtain updated facility contact information. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Health Care Supervisor Audrey Steinbach. Program Director Kendahl Meno arrived later during the visit.

LPA obtained updated phone numbers for the facility and new Director of Client Services (Administrator). LPA provided consultation on the process for submitting documentation for change in Administrator.

LPA did not observe any immediate health and/or safety concerns. No deficiencies were cited during today's visit. An exit interview was conducted with Program Director Meno to whom a copy of this report and was provided. Their signature below confirms receipt of this document.
