Facility Evaluation Report

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted a Case Management visit to cite a deficiency disclosed during a complaint visit. LPA met with House Manager, Samantha Martin. LPA discussed the purpose of the case management visit over the telephone with Licensee, Sabrina Snyder.

During staff interviews conducted during today's visit it was disclosed that on there have been several incidents that occurred at the facility involving facility staff that were not reported to CCL as required. There were no reported injuries resulting from these incidents.

Pursuant to the California Code of Regulations, Title 22, Division 6, deficiency is being cited on the attached LIC809D. A plan of correction was developed with Licensee, Sabrina Snyder over the telephone.

An exit interview was conducted with House Manager, Martin to whom a copy of this report, LIC809D Deficiency Report along with Licensee/Appeal Rights (LIC9058 03/22), were provided.
