Facility Evaluation Report

Licensing Program Analyst (LPA) David Roman conducted a case management visit to cite for deficiencies that were observed during an unrelated visit. LPA met with Licensee, Sabrina Snyder, and discussed the purpose of the visit.

During today’s visit, LPA discovered, through staff interview and review of resident records, that resident 1 is receiving hospice in home services. Licensee reported to have communicated with SDRC about the resident receiving hospice services and the facility's willingness to continue to housing the resident. The hospice resident is receiving in home health through hospice care services. LPA informed that the facility is to request in writing a facility hospice waiver and have it be granted in order to retain the resident with hospice services. A deficiency is being cited Per Title 22, Division 6, Chapter 8 of the California Code of Regulations and is listed on an LIC 809-D.

An exit interview was conducted, and this report was discussed with Licensee, Sabrina Snyder. A copy of the report and Licensee/Appeal Rights were provided to the Licensee, and their signature on this form acknowledges receipt of these rights and a copy of this report.
