Facility Evaluation Report
Licensing Program Analyst (LPA) David Roman conducted an unannounced case management visit in conjunction with a complaint visit. LPA D. Roman identified himself to DSP, Elsa Ramirez, and discussed the purpose of the visit.
While conducting an unrelated investigation visit, LPA D. Roman was not able to review the resident records, intake assessments, pre placement referral, physician reports, etc. as the office that contained the resident files was locked. Staff interviews revealed the office is locked and that the Licensee will return to the facility between 2:00 - 2:30pm. LPA informed that resident files are to be made readily available to CCLD.
Deficiency is being cited per Tittle 22 regulations and listed on LIC 809-D. An exit interview was conducted and a copy of this report was provided to DSP, Elsa Ramirez.
