Facility Evaluation Report

Licensing Program Analyst (LPA), Janette Romero made an unannounced visit to conduct a required annual inspection at the facility. LPA was greeted and granted entry by Administrator, John Durkee who was informed of the purpose of the visit. There was (1) staff present and (0) clients present during the visit.

The facility is a one story home with (3) client bedrooms, (2) bathrooms, and attached garage. The physical plant was clean and observed to be in good repair. LPA observed the facility has a (2) supply of perishable foods and (7) supply of non-perishable food items. Medications, cleaning supplies, and knives are secured and locked. There are hygiene supplies and PPE equipment for clients. There are no bodies of water and no fire arms being stored on the premises. The smoke alarms and carbon monoxide detectors were tested and were operational. All passageways were free of obstruction. Last fire drill was conducted on 08/23/2025 which met the department requirements. LPA reviewed all client files and (4) staff files which possessed all required records. LPA reviewed the Record Of Client's/Resident's Safeguarded Cash Resources (LIC405) for the current clients. The facility has a department approved infectious control plan and emergency and disaster plan which were reviewed during the time of the visit.

No health or safety issues were observed and no deficiencies were cited. An exit interview was conducted and a copy of this report was reviewed and provided.
