The inspector’s account

On August 07, 2026, Licensing Program Analyst (LPA), Venus Mixson made an unannounced visit to the facility to conduct the Annual Required visit, and met with Administrator, Caroline Armstrong. LPA introduced herself and stated the purpose of the visit. The facility has a fire clearance to serve six non-ambulatory elderly residents and an approved hospice waiver for two. During today's visit, there was two staff and six residents present. During today's visit, LPA toured the facility, along with Administrator and observed the facility is made up of a single-story home with six resident bedrooms, three Jack and Jill bathrooms, which connect several of the residents bedrooms. LPA observed the kitchen, dining area, living room and laundry room. The resident bedrooms possess the required furnishings per Regulations. LPA observed the bathrooms to have grab bars and non-skid mats in the showers at the time of this visit. LPA did not observe any bodies of water on the premises currently. Indoor and outdoor pathways were free of obstruction, clutter and debris at this time. LPA toured the kitchen and observed the door with a lock leading to the kitchen having been removed and knives are secured in a kitchen drawer. LPA also observed the facility has more than a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications and disinfectants are secured in different locked closets. LPA observed the smoke alarms, carbon monoxide detectors, and LPA observed them to operational. LPA also observed fire extinguishers mounted near the kitchen and living room that were in the green and serviced currently. LPA conducted staff and resident file reviews and found the required documents at the time of this visit. Additionally current staff possess current criminal record clearance, are associated with the facility, and have a valid first aid/CPR certification. POC were cleared with this visit. There were no observable health and safety challenges at the time of this visit. An exit interview was conducted. A copy of this report was reviewed and given to the Administrator, Caroline Armstrong.
