The inspector’s account

On 03/04/2026, Licensing Program Analysts (LPAs) Janette Romero, Kyle Wellington, and Imaculada Vasquez made an unannounced visit to the facility to follow up on an Unusual Incident/Injury Report (LIC 624) submitted by Administrator Huber Barquero reporting that on 02/20/2026 at approximately 11:00 a.m., an anonymous caller reported that Staff 1 (S1) behaved in a hostile manner while at the facility and disclosed confidential company information. The LIC 624 documents S1 was immediately placed on administrative leave pending an internal investigation, and S1 is not permitted to work or enter any of the Nalas Residential Facilities until further notice.

LPAs were greeted and granted entry into the facility by House Manager Rosaura Estrada. Administrator Huber Barquero was phoned and informed of the purpose of LPAs' visit. Administrative Assistant (AA) Garrett Crow arrived to the facility to meet with LPAs. LPAs toured the facility with AA Crow, conducted partial interviews, and obtained copies of pertinent documentation. During the tour, LPAs did not observe S1 to be present in the facility. No immediate health or safety concerns were observed during today's visit and no deficiencies were cited. The facility was observed to have operating utilities and a two-day supply of perishable foods and seven-day supply of nonperishable food items. Additional information is required to determine whether the reported information warrants a deficiency. An exit interview was conducted and a copy of this report and Confidential Names list (LIC 811) were reviewed and provided to AA Crow.
