The inspector’s account

On 6/27/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. Upon arrival, LPA knocked on the facility's front door and did not receive a response. LPA phoned Administrator, Chris Warner who arrived to the facility to meet with LPA. Warner reported there are no clients currently in care and the utilities remain on with the exception of the gas ,which has been manually shut off to avoid any gas leaks/fire incidents.

LPA toured the facility with Warner and did not make any observations to suggest the home is currently occupied. Warner reported at this time, the licensee has not expressed interest in closing the facility. During the tour, LPA observed client bedrooms have the required bedding, furniture, and lighting and the facility has storage room for perishable and non-perishable foods, confidential files, hygiene supplies, towels and linens. Warner tested the smoke alarm/carbon monoxide detector and LPA heard it to be operational. LPA also observed a fire extinguisher in the kitchen that was last serviced on 1/3/2025. There are no bodies of waters on the premises. Indoor and outdoor passageways are free of obstruction. When asked, Warner reported the previous client and staff files have been archived in the administrative office and unavailable for LPA's review during the visit. Per a Facility Evaluation Report dated 3/21/2025, clients were removed from the facility on 3/21/2025. Warner was also advised that ELI INC is in inactive with the Franchise Tax Bureau (FTB) according to the Secretary of State (SOS) website. During the visit, Warner was unable to produce proof ELI INC is active with the FTB. The facility will be cited for the two (2) deficiencies noted above. An exit interview was conducted and a copy of this report and appeal rights were reviewed and provided to Warner.
