The inspector’s account

On 12/23/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver, Evelia Vasquez. Licensee Josie Dizon and Administrator Jake Dizon arrived during the visit. During the visit, LPA observed one (1) care staff present and was informed all five (5) clients were out in the community attending day program. Staff present have a criminal record clearance and are associated with the facility.

LPA toured the facility with Jake. During the tour, LPA observed the facility is made up of a one (1) story home with three (3) client bedrooms, one client (1) bathroom, one (1) staff room, living room, kitchen, two (2) dining rooms, a laundry room and attached garage. Indoor and outdoor pathways were free of obstructions. No bodies of water were observed on the premises. Jake tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen, last serviced on 11/02/2025. LPA reviewed the facility's Fire Drill log noting their last fire drill was conducted on 10/05/2025. Medications are stored in a living room cabinet that is secured with a master lock. Cleaning solutions and disinfectants are secured in a locked cabinet inside the laundry room. LPA observed additional bedding, towels, linen, incontinent and hygiene supplies for the clients. LPA toured the kitchen and observed the facility has a two-day supply of perishable foods and seven-day supply of non-perishable foods, all stored in a safe and healthful manner. LPA reviewed the Record Of Client's/Resident's Safeguarded Cash Resources (LIC 405) for all five (5) clients while Jake counted their physical monies and no discrepancies were discovered. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Licensee Dizon.
