Facility Evaluation Report

Licensing Program Analyst (LPA),Abdoulaye Zerbo conducted an unannounced visit to the facility for a health an safety check. The LPA met with Licensee Katherine Dowden ,informed her of the purpose of the visit and was granted access.

Community Care Licensing (CCL) received a phone call from the facility representative indicating an alleged physical abuse by Staff 1(S1) toward Client 1 (CL1).

LPA's case management included interview with staff , obtaining pertinent documentation and conducting a tour of the facility for a health and safety check. No immediate health and safety concerns were observed during today's visit. Licensee Katherine Dowden informed LPA that an internal investigation is being conducted. LPA was also informed that S1 has been suspended, pending the investigation.

Licensee Katherine Dowden was advised that possible visits and phone interviews will be conducted before a decision is rendered.

An exit interview was conducted and a copy of this report was provided to Licensee Katherine Dowden.
