Facility Evaluation Report
On 5/27/2025, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced case management visit at the facility to follow up on a Special Incident Report (SIR) dated 5/26/2025 reporting on 5/24/2025, Client 1 (C1) alleged Staff 1 (S1) touched their genital areas. LPA met with Administrators, Katherine Dowden and Ivy Kasko who were informed of the purpose of the visit. During the visit, LPA did not observe care staff present and was informed all the clients, including C1, were attending day program.
LPA toured the facility and obtained copies of pertinent documentation. Dowden reported the alleged incident was reported to law enforcement, San Diego Regional Center, and Community Care Licensing in a timely manner. Dowden was interviewed and reported she received the following information from S1 and Staff 2 (S2). On 5/24/2025, S1 was transporting clients, including C1, to a fast-food restaurant during an outing. S1 was in the van with C1 and Client 2 (C2) as they waited for another client to get into the van. S1 and C1 began joking in the car. C1 expressed they were hungry and S1 informed C1 they were going to the fast food restaurant. S1 stated to C1, "You just ate a yogurt. Look, your belly is big" or words to that effect as S1 playfully tapped on C1's stomach once. Clients and staff arrived to the fast food restaurant and C1 did not appear to be visibly distressed. When S1 went to the restroom, C1 reported the allegation to S2 who then informed Dowden. Dowden directed S2 to be present when S1 transported C1 back to the home. S2 reported they did not observe any inappropriate or unusual interactions between C1 and S2 while they transported C1 back to the facility.
