Facility Evaluation Report
On 4/13/22 Licensing Program Analyst (LPA) Javina George made an unannounced visit at the facility for the purpose of conducting a health and safety check. LPA George was greeted and granted entry by staff Diego Vasquez-Lamadrid, and explained the purpose of the visit. Licensee/Administrator Manuel Vazquez arrived shortly after LPAs arrival. At the time of the visit there was (1) staff and (2) clients present.
LPA conducted a tour of the interior and exterior of the facility. The facility was clean and clutter free. LPA observed the facility to have the required amount of food, 2 day supply of perishable and a 7 day supply of non-perishable food items. The facility has an adequate supply of hygiene items and have the required covid postings. LPA George reviewed and was provided with a copy of the facility's staff schedule. The facility was within the required ratio (3:1) at the time of LPAs visit. The second staff is scheduled to come in at 2pm, before the other two clients return from the day program.
LPA George observed both clients present to be in groomed, dressed and in no apparent acute distress. The clients were outside swinging on the swing and walking around the backyard. No health and safety concerns were observed.
An exit interview was conducted, and a copy of this report was provided to Licensee/Administrator Manuel Vasquez.
