Facility Evaluation Report
On 06/01/2026, Licensing Program Analyst (LPA) Janette Romero made an announced visit to the facility for the purpose of the facility’s closure. LPA met with Licensee Manuel Vasquez. Licensee requested to close the facility and surrender the facility's license as part of a settlement he reached with the Department's legal division on 05/22/2026. As a result, LPA scheduled this visit to conduct a final inspection and walkthrough of the facility. Licensee reported that Client 1 (C1) and Client 2 (C2) previously residing in the facility have been relocated by San Diego Regional Center (SDRC) staff in preparation of the facility's closure.
LPA made contact with SDRC Staff Bianca Landgrave who confirmed C1 and C2 secured alternative placement in another licensed board and care facility vendored by SDRC. During today’s visit, LPA did not observe any indications to suggest that the facility continues to house clients. Licensee reported that while preparing to vacate the facility, he was packing his belongings and inadvertently placed the facility's original license in a box. Licensee agreed to mail the facility's original licensee to the Riverside Regional Office as soon as possible. The effective closure date will be 06/01/2026. An exit interview was conducted and a copy of this report and Confidential Names list (LIC 811) were reviewed and provided to Mr. Vasquez.
