Facility Evaluation Report
On 05/23/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management deficiencies visit. LPA met with Licensee Manuel Vasquez and explained the purpose of the visit. LPA was delivering findings for complaint control number 18-AS-20241029084032.
On 10/22/24 client #1 (C1) was observed to have a red mark around their neck. Licensee Vasquez called LPA to report the incident on 10/22/24 and stated that an Unusual Injury/Incident Report would be submitted, however upon further review there was no official Unusual Injury/Incident Report submitted to the department. LPA followed up with Licensee Vasquez requested proof of fax confirmation but there was not one provided.
Based on interview and records review Licensee Vasquez did not follow reporting requirements, and a citation is being issued on the attached 809D.
An exit interview was conducted and a copy of this report, 809D, appeal rights and LIC9098-Proof of Corrections form was reviewed and provided to Licensee Manuel Vasquez.
