Facility Evaluation Report
On 12/16/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to conduct a 1 year required visit and a case management health checks visit. LPA met with Licensee/administrator Manuel Vasquez where LPA explained the purpose of the visit. During today's inspection LPA verified contact information, with no updates needed at the time. There are no waivers or exceptions on file for the facility.
The facility was observed to be clean with the passageways being free of any obstructions. The smoke and carbon monoxide detectors were tested and found to be operable. The emergency disaster drills are being conducted on a monthly basis with the last drill completed on 11/07/25. The hot water was tested and found to be within regulatory limits measuring at 108.4 degrees Fahrenheit. The sharps, medications, and chemicals were observed to be locked and inaccessible to clients in care. The are (2) fully charged fire extinguishers. There are is no guns, ammunition or bodies of water on the premises.
The client bedrooms were observed to have a bed, chest of drawers, and adequate lighting. A review of records was conducted the facility annual fees were due on or before 12/12/25 and have not been paid (technical violation will be issued). LPA provided the balance and PIN should the licensee wish to pay electronically. The governing body was observed to be in good standing. Staff were observed to have criminal record clearance and to be associated to the facility. The administrator was observed to possess a valid administrator certification that expires 10/05/25. The staff files reviewed were observed to have completed ongoing training, and possess CPR certification. Client personal and Incidental funds were reviewed with no discrepancies found. The facility is equipped with flashlights and emergency supplies such as food and water, as well as personal hygiene supplies, and extra linen. The facility was observed to have the required postings, as well as had an adequate food supply (2 day perishable, 7 day non perishable food items). An exit interview was conducted, where a copy of this report LIC811-confidential names list, appeal rights and was reviewed and provided to Licensee/administrator Manuel Vasquez.
