Facility Evaluation Report

On 11/24/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management health and safety checks visit. LPA met with Licensee/Administrator, Manuel Vasquez, and informed him of the purpose today's visit. At time of the visit there was (1) staff and zero (2) client presents. Below are the observations made during today's visit. LPA provided Mr. Vasqzuez with a copy of the LIC200-application as there is a need to update the facility mailing address. The information was completed during today's visit and the facility information will be updated accordingly.

A tour was conducted of the interior and exterior of the facility; the facility was observed to be clean with the passageways being free of any obstructions.

The facility was observed to have operable for the utilities (water, electricity, gas). The medications, chemicals, sharps and other hazardous items were observed to be locked and inaccessible to clients in care.

The food supply was observed to be sufficient, as there was a 2 day supply of perishable and a 7 day supply of non perishable food items. The Personal and Incidental (P&I) funds, and client medications were reviewed with no discrepancies found. In regards repairs to the facility, Mr. Vasquez is in the process of replacing a minimum of three (3) wooden planks on the back patio. LPA observed for there to be wood that was chipped at the ends of the board. They are covered with a rubber mat to prevent any injuries. There were no health or safety concerned were observed during the time of the visit.

An exit interview was conducted where this report was reviewed and provided to Licensee/Administrator Manuel Vasquez.
