The inspector’s account

On 10/29/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management health and safety checks visit. LPA met with Licensee/Administrator, Manuel Vasquez, and informed him of the purpose today's visit. At time of the visit there was (1) staff and zero (2) client presents. Below are the observations made during today's visit. LPA verified facility contact information that is on file with the department is up to date.

A tour was conducted of the interior and exterior of the facility; clean with the passageways being free of any obstructions. The medications, chemicals, sharps and other hazardous items were observed to be locked and inaccessible to clients in care.

The facility was observed to have operable for the utilities (water, electricity, gas). The facility food supply was observed to be sufficient, as there was a 2 day supply of perishable and a 7 day supply of non perishable food items, there was no expired food items observed. The Personal and Incidental (P&I) funds, and client medications were reviewed with no discrepancies found.

There were no health or safety concerned were observed during the time of the visit.

An exit interview was conducted where this report was reviewed and provided to Licensee/Administrator Manuel Vasquez.
