Facility Evaluation Report
On 09/22/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management health and safety checks visit. LPA met with Licensee/Administrator, Manuel Vasquez, and informed him of the purpose today's visit. Initially at time of the visit there was (1) staff and zero (2) client presents.
LPA obtained a copy of the facility staff schedule, as well as conducted a review of the food supply log, and hazardous item logs.
A tour was conducted of the interior and exterior of the facility; clean with the passageways being free of any obstructions. The medications, chemicals and other hazardous items were observed to be locked and inaccessible to clients in care. LPA observed for the utilities (water, electricity, gas) to be operable. The facility food supply was observed to meet the minimum, as there was a 2 day supply of perishable and a 7 day supply of perishable food items. The Personal and Incidental (P&I) funds, and client medications were reviewed with no discrepancies found.
There were no health or safety concerned were observed during the time of the visit.
An exit interview was conducted where this report was reviewed and provided to Licensee/Administrator Manuel Vasquez.
