The inspector’s account

On 08/25/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management health and safety checks visit. LPA met with Licensee/Administrator, Manuel Vasquez, and informed him of the purpose today's visit. Initially at time of the visit there was (1) staff and zero (0) client presents. The clients then arrived day program at 2:30pm. Below are the observations made during today’s visit.

The utilities: electricity, water, and gas were observed to be operable. The fire extinguishers were checked and were last serviced on 03/28/25. The facility was observed to be clean with the passageways being free of any obstructions. The food supply was observed to be adequate, as there was a 2 day supply of perishable and a 7 day supply of perishable food items. The medications, chemicals and other hazardous items were observed to be locked and inaccessible to clients in care. The Personal and Incidental (P&I) funds, were reviewed with no discrepancies found. As well as the medications were reviewed with no discrepancies found, as evidenced by the Medication Authorization Record (MAR).

Additionally during today's visit, LPA obtained a copy of the facility staff schedule, reviewed the personnel roster and verified contact information, as well as conducted a review of the food supply log, and hazardous item logs.

There were no health or safety concerned were observed during the time of the visit.

An exit interview was conducted where this report was reviewed and provided to Licensee/Administrator Manuel Vasquez.
