The inspector’s account

On 07/25/25 Licensing Program Manager (LPM) Carolyn Tuba and Licensing Program Analyst (LPA) Javina George made an unannounced case management health and safety checks visit. LPA met with Licensee/Administrator, Manuel Vasquez, and informed him of the purpose today's visit. At the time of the visit there are (1) staff and (2) clients present. Below are the observations made during today’s visit.

LPA obtained a copy of the facility staff schedule, reviewed the personnel roster and verified contact information, as well as conducted a review of the food supply log, and hazardous item logs.

The facility was observed to be clean with the passageways being free of any obstructions. The utilities: electricity, water, and gas were observed to be operable. The food supply was observed to be adequate, and to meet the minimum staple requirement for perishable and nonperishable food items. The medications, chemicals and other hazardous items were observed to be locked and inaccessible to clients in care. The Personal and Incidental (P&I) funds, were counted and there was no discrepancies found. LPA discussed the facility annual fees that are now considered past due as they were due in December 2024. Per Licensee the fees will be paid and proof will be submitted to LPA no later than 08/20/25.

There were no health or safety concerned were observed during the time of the visit.

An exit interview was conducted where this report was reviewed and provided to Licensee/Administrator Manuel Vasquez.
