Facility Evaluation Report

On 04/28/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management health and safety checks visit along with Service Coordinator Bianca Landgrave. LPA met with Licensee, Manuel Vasquez, and informed him of the purpose today's visit. At the time of the visit there are (1) staff and (2) clients present. LPA observed the following during today's visit: A tour of the interior and exterior was conducted, and was found clean and the passageways were free from obstruction.

Client medications were reviewed and the new cycle was has been received, and the Medication Authorization Record (MAR) has been completed.

The facility utilities such as gas, water and electric were observed to be operable, and the food supply was observed to be adequate, as there was a 2 day supply of perishable and a 7 day supply of non perishable food items.

LPA conducted a review of the following documents: Personal and Incidental (P&I) Funds were counted and matched the amount noted on the Client Cash Resource Log. LPA obtained a copy of the facility staff schedule, as well as reviewed the food supply log, and hazardous item logs that were observed to be up to date verifying that staff were conducting internal compliance checks. Additionally, during today's visit a review of Client #1 (C1)s continuation of services, Choice of service and standardized annual review was reviewed and signatures were obtained, with Service Coordinator Landgrave.

There were no health or safety concerned were observed during the time of the visit.

An exit interview was conducted where this report was reviewed and provided to Licensee/Administrator Manuel Vasquez.
