Facility Evaluation Report
On 07/22/24 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a health and safety check. At the time of the visit was one (1) staff and (3) client's present. Client #1 (C1) was resting and laying down in bed. Client #2 (C2) was outside, and Client (C3) was sitting in their room, watching a program.
During today's health and safety check LPA conducted an overall status of what has been going on at the facility in regards to any changes since LPAs visit last month. Licensee Manuel Vazquez informed LPA that C1 will be moving out of the facility, as a 30 day notification was received today 07/22/24. In addition a review of the facility's Technical Assistance Plan dated 7/15/24 was reviewed. The plan was created to support the facility and to mitigate any future substantial inadequacies. The plan includes the following:
-making any necessary repairs to the deck
- Home and Community Based Services (HCBS) final rule training
-Personal and Incidental cash training (P&I)
- a meeting with the treatment team in regards to the window and safety vinyl covers
The medications are locked and inaccessible to the residents in care. Medications were reviewed and present and are being given according to the physician's instructions per the Medication Administration Record (MAR).
During today's health and safety check LPA observed for the facility food supply to be sufficient as there was a 2 day supply of perishable and a 7 day supply of nonperishable food items.
The facility was observed to have operable utilities (gas, water, electricity). The facility has a supply of hygiene products such as soap and shampoo for the residents to use. The chemicals are stored in a cabinet inside the garage that was observed to be locked and inaccessible to residents in care.
An exit interview was conducted, and a copy of this report, was provided to the Licensee/Administrator, Manuel Vasquez.
