Licensing Program Analyst (LPA) Javina George and Bianca Landgrave, Service Coordinator from San Diego Regional Center conducted a health and safety check. At the time of the visit was one (1) staff and (3) client's present.

During today's health and safety check LPA observed for the facility food supply to be sufficient as there was a 2 day supply of perishable and a 7 day supply of nonperishable food items. LPA observed for there was a half bag of mixed vegetables that was discarded during LPAs visit as it was expired.

The facility was observed to have working utilities (gas, water, electricity). The medications are locked and inaccessible to the residents in care. The facility has a supply of hygiene products such as soap and shampoo for the residents to use. The chemicals are stored in a cabinet inside the garage that was observed to have the lock on but not fully closed/engaged.

LPA discussed with Licensee Vazquez the importance of double checking and having all locks fully engaged. Per the Licensee the clients do not go into the garage without staff, and the only time is when the client's do enter the garage is when they are washing clothes.

Licensee Vazquez shared that the Home and Community Based Services (HCBS) Final Rule certification is being held tomorrow 6/19/24. During this time a review of the facility program design, locks on door knobs, admission's agreement as well as house rules will be modified to exhibit friendly language.

An exit interview was conducted, and a copy of this report, form was provided to the Licensee/Administrator, Manuel Vasquez.
