The inspector’s account

On today's date 3/29/24 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a health and safety check. LPA met with Licensee/Administrator Manuel Vazquez and explained the purpose of the visit.

At the time of the visit there was zero (0) residents and one (1) staff present. Both Resident #1 (R1) and Resident #2 (R2) were at the day program at the time of LPAs visit. Resident #3 (R3) is from the sister facility #374604100 that was at the day program during LPAs visit but continues to alternate between both homes, to assist with staffing ratios, and current staffing challenges. Per the Licensee (R3) moving to the home on a permanent basis is in process, they have an upcoming annual meeting with the treatment team, to finalize the the moving date.

LPA observed for the facility to still have a sufficient food supply have a two day supply of perishable and a seven day supply of non-perishable food items.

The facility was observed to have working utilities (gas, water, electricity). LPA observed for the new cycle of medications to have been received and logged. The medications are locked and inaccessible to the residents in care.

Inside the bathroom, LPA observed for the linoleum to be lifting around the toilet seat. Per Licensee Vazquez R3 is picking the linoleum as well as the paint on the cabinet inside the bathroom. Per Licensee Vazquez he will make the necessary repairs, and send the proof of repair to LPA no later than Saturday 3/30/24.

An exit interview was conducted, and a copy of this report was provided to the Licensee/Administrator, Manuel Vasquez.
