The inspector’s account

On 1/31/24 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a health and safety check. LPA met with Licensee/Administrator Manuel Vazquez and explained the purpose of the visit.

At the time of the visit there was two (2) clients and one (1) staff present. Client #1 (C1) was inside their bedroom. Client # 2 (C2) had not returned from the day program at the time of LPAs visit. Client # 3 (C3) is from the sister facility #374604100 and observed to be completing a puzzle. The facility was observed to be within the licensed capacity (4). The facility has (3) rooms that are vacant, and (2) rooms that are occupied.

During today's health and safety check LPA verified, that the necessary repairs are still in tact in the bathroom (leaking pipe underneath the sink). Please tote that LPA was sent proof of correction on 12/28/23, which was the same day, the facility was noted to repair the leaking pipe. Licensee shared that upon the repair there was an additional leak that was discovered underneath the home, with the assistance with the water company. Licensee provided proof of the repaired pipe.

LPA observed for the facility food supply to be sufficient, as LPA observed for the facility to have a two day supply of perishable and a seven day supply of non-perishable food items. The facility continues to have a sufficient amount of personal protective equipment (PPE) supplies at the facility, and a sufficient amount of paper supplies (paper towels and toilet paper), for the amount of residents in care.

The facility was observed to have working utilities (gas, water, electricity). LPA observed for the medications to be locked and inaccessible to the residents in care. There were no citations issued during today's visit, as LPA did not observe any health and safety concerns.

An exit interview was conducted, and a copy of this report was provided to the Licensee, Manuel Vasquez.
