Facility Evaluation Report
On 6/14/22 Licensing Program Analyst (LPA) Javina George made an unannounced visit at the facility for the purpose of conducting a health and safety check. LPA George met with Diego Vazquez-La Madrid, DSP, and explained the purpose of the visit.
At the time of the visit there was (2) staff and (4) clients present. LPA observed the facility to be within the required ratio of 3:1. LPA conducted a tour of the facility, the facility was clean and clutter free. All clients were observed eating dinner.
LPA observed the facility's food supply which met the requirements; 2 day supply of perishable and a 7 day supply of non-perishable food items.
No citations were issued health and safety concerns were observed during the visit,
An exit interview was conducted, and a copy of this report was provided to Diego Vazquez-La Madrid, DSP.
