Facility Evaluation Report
On 5/17/22 Licensing Program Analyst (LPA) Javina George made an unannounced visit at the facility for the purpose of conducting a health and safety check. LPA George met with DSP Diego Vazquez-LaMadrid, and explained the purpose of the visit.
At the time of the visit there was (1) staff and (2) clients present. LPA observed the facility to be within the required ratio of 3:1. LPA conducted a tour of the facility, the facility was clean and clutter free. The two clients present at the facility Clients #1 and #2 (C1 and C2), were observed to be asleep inside of their bedrooms. C3 and C4 were at the day program.
LPA observed the facility's food supply which met the requirements; 2 day supply of perishable and a 7 day supply of non-perishable food items.
No health and safety concerns were observed during the visit, therefore no citations were issued.
An exit interview was conducted, and a copy of this report and LIC811 was provided to Diego Vazquez-LaMadrid.
