Facility Evaluation Report

On 03/25/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced case management visit to assess for any health or safety concerns. LPA met with Licensee Manuel Vasquez who was informed of the purpose of the visit. Licensee Vasquez reported this facility has not housed any clients for approximately the past three (3) years.

During the visit, LPA toured the facility with Licensee Vasquez and did not observe any clients present or evidence to suggest the home is currently occupied. The facility was observed to have operable utilities including gas, water, and electricity. No immediate health or safety concerns were observed during today's visit. Licensee Vasquez was advised to notify LPA and Community Care Licensing if he is ever made aware of any new potential client placement. An exit interview was conducted and a copy of the report was reviewed and provided to Licensee Vasquez.
