The inspector’s account

On 06/20/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct an annual inspection and health and safety check. LPA met with Licensee/Administrator, Manuel Vasquez and explained the purpose of the visit. The home remains open, however there are no clients in care. The facility is licensed to serve 4 ambulatory clients age ranges 18-59.

The facility is a single story structure consisting of four (4) bedrooms, two (2) bathroom home, with a sensory room, and backyard. LPA observed all toxic chemicals and other hazardous items to be locked and inaccessible to clients and are centrally stored in a locked cabinet in the garage. The facility currently has a 7 day supply of non-perishable food items, Licensee Vasquez acknowledged that the facility is to be prepared to have a 2 day supply of perishable food items, as soon as there have been notification of any client placement's.

There were no records reviewed of clients, including personal and incidental (P&I) funds or medications available to review as there are no clients at the home. The smoke and carbon monoxide detectors were tested and found to be operable. There are no pools or bodies of water, or no known guns or ammunition on the premises. A records review of Licensee/administrator Vasquez's file was conducted that revealed Vasquez to have valid CPR/first aid certification that expires on 06/18/26. Administrator certification renewal was observed to be pending for Vasquez as of 02/20/25. Per a file review conducted the facility annual fees have not been paid since 2023, LPA provided guidance as well as the PIN 252035, should the Licensee wish to pay electronically. Per Vasquez the fees will be as soon a determination is made from a pending legal matter.

An exit interview was conducted and a copy of the report was provided to Licensee Manuel Vasquez.
