Facility Evaluation Report
On 6/14/22 Licensing Program Analyst (LPA) Javina George made an unannounced visit at the facility for the purpose of conducting a health and safety check. LPA George met with Jalisa Fore, DSP, and explained the purpose of the visit.
LPA observed the facility to be within the required ratio of 3:1, as the time of the visit there was (1) staff and (2) clients present. LPA conducted a tour of the facility, the facility was clean and clutter free. All clients were inside the home. 1 client was watching a movie in the sensory room and 1 was inside their bedroom watching a program about birds.
LPA observed the facility's food supply which met the requirements; 2 day supply of perishable and a 7 day supply of non-perishable food items.
No citations were issued health and safety concerns were observed during the visit,
An exit interview was conducted, and a copy of this report was provided to Jalisa Fore, DSP.
