Facility Evaluation Report

On 5/26/22 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility for the purpose of conducting a health and safety check. LPA George met with DSP Jaciel, and explained the purpose of the visit.

At the time of the visit there was (1) staff and (2) clients present. LPA observed the facility to be within the required ratio. Both clients were receiving their PM medications, and had showered and were dressed for bed.

LPA conducted a tour of the facility, the facility was observed to be cleaned and clutter free. The facility was observed to have met the food requirements, by having a 2 day supply of perishable and a 7 day supply of non-perishable food items.

There were no health and safety concerns observed, therefore no citations were issued during today's visit.

An exit interview was conducted, and a copy of this report was provided to Jaciel Ordaz, DSP.
