Facility Evaluation Report
On 5/17/22 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility for the purpose of conducting a health and safety check. LPA George met with Licensee/Administrator Manuel Vazquez, and explained the purpose of the visit.
At the time of the visit there was (1) staff and (2) clients present. LPA observed the facility to be within the required ratio of 3:1. Both clients were observed to be asleep inside of their bedrooms.
LPA conducted a tour of the facility, the facility was observed to be cleaned and clutter free. The facility was observed to have met the food requirements, by having a 2 day supply of perishable and a 7 day supply of non-perishable food items.
There were no health and safety concerns observed, therefore no citations were issued during today's visit.
An exit interview was conducted, and a copy of this report was provided to Licensee/Administrator Manuel Vasquez.
