On 12/15/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by House Manager (HM), Juan Arguello who was informed of the purpose of the visit. The facility has a fire clearance to serve four (4) adults, of which two (2) may be non-ambulatory. During today's visit, there was three (3) staff and one (1) client present and LPA was informed three (3) clients were out in the community attending day program. LPA toured the facility with HM Arguello and observed the facility is made up of a one-story home with four (4) client bedrooms, two (2) bathrooms, a kitchen, two (2) dining areas, two (2) living rooms, a laundry room, and attached garage. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had soap and toilet paper readily available. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility met Departmental requirements for a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are secured in a locked kitchen cabinet. LPA and HM reviewed all clients' medication administration records along with their physical medications and no discrepancies were discovered. Chemicals and cleaning solutions are secured in a locked laundry room cabinet and storage room in the garage. LPA observed additional hygiene supplies and clean bedding available in different colors. HM tested multiple smoke alarms/carbon monoxide detectors and LPA heard them to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen last serviced on 05/15/2025.
