The inspector’s account

On 09/03/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by House Manager (HM), Sonia Villegas who was informed of the purpose of the visit. Administrator, Dara Crow was contacted over the phone and also informed of the purpose of LPA's visit. The facility has a fire clearance for six clients of which one (1) may be non-ambulatory. During the visit, there was one (1) staff present and LPA was informed all five (5) clients were attending day program. The staff present had a criminal record clearance and was associated with the facility.

LPA toured the facility with HM Villegas. During the tour, LPA observed the facility is made up of a one (1) story home with five (5) client bedrooms, two (2) bathrooms, a staff room, two (2) living rooms, a kitchen, dining room and attached garage. All client bedrooms had the required bedding, furniture and lighting. Bathrooms had a working toilet and wash basin. Indoor and outdoor pathways were free of obstructions.. No bodies of water were observed on the premises. Staff tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen last serviced on 05/15/2025. LPA reviewed the facility's Fire Evacuation Drill Log noting their last fire drill was conducted on 08/31/2025. LPA reviewed the Record Of Client's/Resident's Safeguarded Cash Resources for all five (5) clients while HM Villegas counted their physical monies and no discrepancies were discovered. Medications are secured in a locked hallway closet. LPA toured the kitchen and observed the facility has a two-day supply of perishable foods and a seven-day supply of non-perishable foods, which are stored in a safe and healthful manner. Cleaning solutions and disinfectants are secured in cabinet above the laundry service. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to HM Villegas.
