Facility Evaluation Report
On 4/16/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Executive Director (ED) Anand Mehta and Resident Services Director (RSD), Melissa Mortford who were informed of the purpose of the visit. The social rehabilitation facility has a fire clearance to serve six (6) ambulatory clients. During the visit, there was five (5) clients and four (4) staff present.
LPA toured the facility with RSD and observed the facility is made up of a two-story home with three (3) client bedrooms, six (6) restrooms, several administrative offices, a kitchen, dining room, living room, laundry room, and attached garage. The facility has an in-ground pool and spa, which are covered with a pool net to form a safety barrier. Indoor and outdoor pathways were free of obstruction. ED tested one (1) of the smoke alarms/carbon monoxide detectors and LPA found them to be hardwired and operational. LPA also observed several charged fire extinguishers mounted throughout the facility. LPA toured the kitchen and observed the facility had more than a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are secured in a locked cabinet in the locked medication room located near the living room. LPA toured the garage and observed an emergency supply of food and water. Client files reviewed had updated and signed physician's reports, admission agreements, and needs and services plans. Staff present have a criminal record clearance and are associated with the facility. The facility's last fire drill was conducted on 3/28/2025. The facility's certificate of liability insurance expires on 11/1/2025. Clients' personal rights, complaint procedures, emergency disaster plan/phone numbers, and facility sketches are visibly posted near the front entrance.
During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to ED Mehta.
