Facility Evaluation Report

Licensing Program Analyst (LPA) Sara Martinez made an unannounced visit to the facility. The purpose of the visit was to conduct interviews with staff regarding a complaint that is related/associated with this facility.

LPA was greeted and granted entry by Priscilla Bermudes Resident Services Director, where LPA explained the purpose of her visit.

No heath and safety concerns were observed at the time of LPAs visit.

An exit interview was conducted, and a copy of this report was reviewed and provided to Bermudes.
