Facility Evaluation Report
On 5/5/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required inspection. LPA met with Administrator, Mirjana Radjenovic, where LPA explained the purpose of the visit. At the time of the visit there was (1) Staff and (0) clients, as the clients were at the day program. The home is licensed to serve 6 ambulatory mentally disabled adults. The home is not vendorized with Regional Center. Below is what was observed during today's visit:
The home is a two story structure, however the first half/right side of the house on the downstairs level is designated for the clients. There is a total of (3) bedrooms and (2) bathrooms. There are no pools or bodies of water on the premises. The combined smoke and carbon monoxide detectors were tested and found to be operable. The fire extinguisher was last checked in March 2025, was fully charged with the tag in tact. The hot water was tested and found to be within regulatory limits measuring at 115.8 degrees Fahrenheit. The medications were observed to be locked inside the closet in the hallway across from the laundry room. The facility is conducting drills on a quarterly basis and the last drill was conducted on 4/29/25.
The facility food supply was observed to be adequate as the staple of a 2 day supply of perishable and 7 day supply of non perishable food items was observed. There was an expired package of pasta that was discarded during LPAs visit. Records review: the client files possessed appraisals and medical assessments, no personal and incidental (P&I) funds were reviewed as the clients do not received P&I. The staff files were observed to have valid CPR training, that expires on 6/8/25. In addition the administrator Mirjana Radjenovic was observed to possess a valid administrator certificate which expires on 4/8/26. The facility annual fees are due on or before 5/27/25, LPA provided PIN 507587, should the Licensee wish to pay electronically. Based on today's inspection no deficiencies were observed. An exit interview was conducted and a copy of this report was reviewed and provided to Mirjana Radjenovic, Administrator.
