Facility Evaluation Report
On 12/15/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit. LPA met with staff Wilfredo Zamora and explained the purpose of the visit. LPA was granted entry and toured the facility alongside Wilfredo. LPA observed the following during the visit:
The facility is a single-story structure which consist of (4) four client bedrooms, (2) two bathrooms, (1) staff bedroom, living room, dining room, kitchen, garage, and backyard. Client bedrooms were equipped with the required bedding, furniture, and functional lighting. Bathrooms were kept clean and sanitary. LPA observed a hallway closet that stored additional linen and towels. All indoor/outdoor passageways were kept free from obstruction. Carbon monoxide and smoke detectors were tested and were observed to be fully operable. Kitchen area was kept free of debris and vermin. Knives and other sharp items were stored in a locked cabinet. Centrally stored medication were also observed in a locked cabinet located in the kitchen. Facility maintained a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. During the time of visit, all clients were attending day program. Per Administrator Rada Roksandic, there are no firearms and/or ammunition on the premises.
LPA observed preplacement referrals, current IPP's, admission agreements, weight charts, physician reports, and medication administration records. Staff record review included but not limited to background clearance, health screening, personnel records, relevant training's, and valid CPR/First-Aid Certification. LPA did not observe any health and safety concerns.
An exit interview was conducted and a copy of the report was provided to Administrator, Rada Roksandic.
