Facility Evaluation Report
On 3/25/26, Licensing Program Analyst (LPA) Kyle Wellington made an unannounced visit to the facility to conduct an annual inspection. LPA met with Administrator (Admin), Gillian Hennessey who was informed of the purpose of the visit. The facility has a fire clearance for a total capacity of 60 adults of which 50 may be non-ambulatory. During the visit, Admin reported there was 44 clients and 22 staff present. Admin informed LPA that the facility is a licensed vendor of the San Diego Regional Center. LPA received a resident and staff roster from the Admin. LPA did an observation of the inside of the facility with the Admin and conducted record reviews for the inspection.
LPA toured the facility with the Admin. The facility is made up of three (3) classrooms, five (5) restrooms, several staff offices, a kitchen/dining room and a large multipurpose room. LPA observed the kitchen/dining room to be clean. Admin informed LPA that clients provide their own lunches and snacks to consume during day program hours. LPA observed restrooms to be clean and sanitary with toilet paper and incontinent supplies along with soap and paper towels readily available. Hand sanitizer was available throughout the facility. Admin informed LPA that one client receives medication at the facility. LPA observed the medication to be secured in a medication lock box stored in one of the staff offices. LPA reviewed the Medication Administration Record (MAR) for the client. LPA reviewed the facility’s System Record of Inspection and Testing (SRIT) conducted by Siemens Industry, Inc noting the fire alarm system was inspected on 11/24/25 and found to be functioning properly. LPA reviewed the facility’s Record of Evacuation Training noting the facility’s last fire drill was conducted on 2/13/26. LPA observed several fire extinguishers charged and mounted throughout the facility that were last serviced on 3/17/26. LPA observed that all facility exits were clear of obstructions and had the required signage. LPA reviewed several staff and clients files. Staff present have a criminal record clearance on file.
