Facility Evaluation Report
On February 24, 2026, Licensing Program Analysts, (LPA) Mia Lankford, Adboulaye Zerbo LPA and Ahlilah sharp LPA arrived at the facility unannounced to conduct the Required Annual Inspection and met with the Caregiver, Romina Mas. The facility file review was conducted and additional records were requested and reviewed on site. The facility is licensed for four (4) Adults and is currently operating at a capacity of three(3) adults.
LPA's toured the facility along with the Caregiver, Romina Mas and made observations pertaining to the annual visit. LPA's inspected the facility inside and outside there were no obstructions or debris to the indoor or outdoor passageways at the time of this visit. Additionally, there were no bodies of water on the premises. The facility is comprised of a single story home with for four (4) clients bedroom, storage, kitchen, laundry room staff room and detached garage.
Physical Plant: LPA's observed the three resident’s bedrooms, and each was fully furnished . LPA's inspected the facility bathrooms, and the hot water temperature tested within regulations at 112.4 degrees. The bathrooms were clean, and appliances were operating appropriately at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers expire on 10/1/2026. LPA's observed the signs "Personal Rights," and the LTCO poster in Living room. The cleaning supplies and sharp items were locked and inaccessible to the clients in care presently. There were designated storage spaces for the clients and staff’s files, and this file cabinet was locked and inaccessible to clients in care at the time of this annual visit.
