The inspector’s account

On 11/18/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required annual inspection. LPA met with staff, Alberto Vera, and explained to Alberto the purpose of the visit. LPA Flores was granted entry and conducted a tour of the interiors/exteriors areas of the facility. LPA Flores observed the following during the visit:

During the inspection, (6) six out of (6) six clients were out in the community, attending day program. The facility is a single-story structure which consists of (4) four client bedroom, (2) two bathrooms, (1) one staff bathrooms, (2) two living rooms, dining room, kitchen, and garage. The facility kitchen was kept clean and free of litter. The facility maintained a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. Client bedrooms met the required bedding, seating, furniture, and functional lighting. Indoor/outdoor passageways were free from obstructions. LPA observed activities in the living room to promote group socialization. The medications are stored in a locked cabinet in the living room that are inaccessible to clients. Smoke and carbon monoxide detectors were tested and were observed to be fully operable. Water temperature measured at 105.2 degrees Fahrenheit. Per Administrator Susan Beal, there are no firearms and/or ammunition on the premises.

LPA observed the See Something Say Something, Client Personal Rights, facility sketch, and license posted on the wall near the kitchen. During client records review, LPA observed preplacement referrals, current IPP's, admission agreements, weight charts, physician reports, and medication administration records. Staff record review included but not limited to background clearance, health screening, personnel records, and valid CPR/First-Aid Certification. LPA did not observe any health and safety concerns.

An exit interview was conducted and a copy of the report was provided to Administrator, Susan Beal.
