Facility Evaluation Report
On 02/12/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA met with Administrator Brianda Munoz where LPA explained the purpose of the visit. Note that there was a change of administrator request, that will be made. At the time of the visit there was (7) clients and (4) staff present. All staff present obtained proper criminal record clearance and were associated to the facility. LPA verified facility contact information, and will update the information accordingly. The facility is licensed to serve (8) neurologically disabled adults ages 18-59, of which 4 may be non ambulatory. The facility is a multi level structure consisting of (4) levels with an activity room, game room, gym, elevator, kitchen, panty, cleaning supply closets, laundry rooms, and staff offices-4th floor including a storage room with hygiene items. The facility is utilizing video surveillance in the common areas of the facility as well as the exterior. The clients have signed an acknowledgment form, and the facility sketch was updated, just need to posted. The client bedrooms were observed to have the required furniture and were observed to be clean. In addition the bathrooms were observed to be clean. The water temperature was tested and needed to be readjusted as it measured to be 127.0 degrees Fahrenheit. The chemicals and other hazardous items were observed to be locked and inaccessible to clients in care. The medications are stored in a locked medication cart on the second floor. The facility uses an EMAR system to keep track of client medications.
The facility was observed to be clean and clutter free. The food supply was sufficient as there was a 2 day supply of perishable and a 7 day supply of non perishable food items. The client files were reviewed and were observed to have the required documentation such as assessments including medical. Staff files were observed to have the required training including CPR/First aid as well as water safety. The emergency disaster drills are being conducted on a quarterly basis and the last drill conducted was on 11/25/24. The facility has (5) fire extinguishers that were last serviced 11/21/24. There are no known guns or ammunition on the premises. The facility was observed to have operable smoke and carbon monoxide detectors. The facility has a pool that was secured by a locked fence. Based on today's inspection, no citations were issued. An exit interview was conducted and a copy of this report was reviewed and provided to Brianda Munoz, Administrator.
