Facility Evaluation Report

Licensing Program Analyst (LPA) Jacqueline Shaw-Ross arrived unannounced at the facility to conduct a case management visit over incidents that occurred on 01/16/2026 and 01/17/2026 at the facility. This is an additional visit to the case management visit that was conducted on 01/22/2026. LPA met with Administrator Maribel Robledo and the purpose of the visit was explained.

A tour of the facility was conducted and no immediate health and safety concerns were observed during the visit. LPA conducted additional interviews with staff and residents, and obtained pertinent copies of documents related to the incidents. The Administrator informed LPA that all residents are doing fine and are being well cared for. No other SIRs related to the incidents have occurred or been reported.
