Facility Evaluation Report
On 01/22/2026, Licensing Program Analyst (LPA) Jacqueline Shaw-Ross arrived to the facility to conduct a case management visit. The department received two special incident reports on 01/21/2026, alleging sexual abuse by a staff member involving a resident. The incidents were self reported by the facility's Administrator. LPA introduced herself to the Administrator and explained the purpose of the visit. A tour of the facility was conducted. Present at the facility were two staff members and the Administrator. LPA was informed all residents were out at day program.
LPA interviewed the administrator and staff member who witnessed the incident. LPA was advised the staff member has been placed on suspension until further notice. LPA obtained documents related to two incidents that occurred on 01/15/2025 and 01/16/2026. LPA advised that additional information and interviews may be needed before concluding the investigation.
Based on today's visit no or health and safety concerns were observed.
An exit interview was conducted and a copy of this report was provided to Maribel Robledo, Administrator.
