Facility Evaluation Report

On 11/14/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required inspection. LPA met with Administrator Maribel Robledo where LPA explained the purpose of the visit. At the time of the visit there was (1) staff and (0) clients present. A file review was conducted and revealed that the facility annual fees that were due on or before 11/13/25, had not been paid during today's visit LPA inquired and the invoice had not been received, LPA provided PIN 460993 , as well as the LIC 200-application, so the mailing address can be updated as needed. Note that the facility annual fees were paid during LPAs visit. The facility removed the awnings that were in the backyard and is currently painting the backyard.

LPA conducted a tour of the interior/exterior areas of the facility. The facility is equipped with solar panels. The facility was observed to be clean and clutter free with the passageways being free of obstruction. The facility was observed to have required postings. The client bedrooms had the required furniture, bed, chair, lamps, chest of drawers.

The smoke and carbon monoxide detectors were tested and found to be operable. The fire extinguishers were fully charged and last serviced on 11/07/25. The hot water temperature was tested and found to be within regulatory limits, measuring at 112.2. The facility is conducting emergency disaster drills on a monthly basis with the last drill being conducted on 11/02/25. There are no pools or bodies of water nor guns or ammunition on the premises. The food supply was observed to be adequate as there a 2 day supply of perishable and a 7 day supply of nonperishable food items.
