Facility Evaluation Report
On 09/18/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and granted entry by Caregivers Jamie and Ludivina Frogoso, where LPAs explained the purpose of today's visit. At the time of the visit there was (2) staff and (1) clients present. Below are the observations made during today's visit: The facility annual fees were paid.
The facility was observed to be clean, with the passageways being free of obstructions. The emergency disaster drills are being conducted on a quarterly basis with the last drill being on 09/06/25. The combined smoke and carbon monoxide detectors were tested and found to be operable. The medications, chemicals and other hazardous items were observed to be locked and inaccessible to clients in care. The are no known guns on the premies in addition to no pools or bodies of water. The hot water was tested and was found to be too hot. The water temperature was adjusted, during LPAs visit retested and it was 109 degrees Fahrenheit.
The facility food supply met the requirements of a 2 day supply of perishable and a 7 day supply of nonperishable food items. A records review of both staff and client files was conducted. Client files were observed to have the required documentation such as admissions agreement, medical assessment, Individual Program Plan (IPP). Staff present at the facility were observed to have obtained criminal record clearance, and to be associated the facility. In addition to having valid CPR certification, and to have receive annual/ongoing training.
An exit interview was conducted and a copy of this report, appeal rights, LIC 9098-Proof of Corrections form was provided to Ludivina Frogoso, Caregiver.
