On 05/23/25 Licensing Program Analyst (LPA) Javina George conducted an unannounced case management deficiencies visit to the facility.
LPA conducted a file review and observed for the facility administrator Emma Imson to have an expired administrator certification. Administrator Imson's certificate #6051610735, expired on 03/26/25.
Per conversation with Administrator Imson, she has twenty hours of CEUs to complete. As a result a deficiency is being issued on the attached 809D.
An exit interview was conducted and a copy of this report, 809D, appeal rights, LIC9098-Proof of Corrections form was reviewed and provided to Emma Imson.
