Facility Evaluation Report
On 04/22/25 at 4pm Licensing Program Analyst (LPA) Javina George conducted an unannounced case management deficiencies visit to facility. LPA met with Administrator Emma Imson, where LPA explained the purpose of the visit.
During today's visit LPA observed for Staff #1 (S1) to have obtained criminal record clearance but is not associated to the facility. Per an interview via telephone with Administrator Emma Imson, she stated that she forgot to send in the request to have S1s fingerprints transferred to the facility. $500 in Immediate civil penalties are being assessed.
During today's visit Administrator Emma Imson provided a LIC9182 and a copy of valid identification for S1, so that they may be associated to the home using the Guardian system.
An exit interview was conducted and a copy of this report, 809D, appeal rights, LIC9098-proof of Corrections forms, LIC421BG, LIC811-confidential names list was reviewed and provided to Administrator Emma Imson.
